ACES IT is happy to assist our off-site locations with retired equipment removal. We ask that your county/site help us in this transition by filling out the appropriate paperwork for removal or transfer of equipment and our staff will transport your equipment.
Inventory Change Request Form
Use this form when sending surplus items to Property. Please fill out the required details and email it to the ACES IT Help Desk. Departments should NOT sign these forms. ACES IT will obtain the required college-level signature (Section 3: Official Approval) once the items arrive at our office and are ready for transfer.
